Complete the SAPS 520 Form and CBP form 4457_0 A copy of your itinerary from your travel agent. Travel to South Africa: A valid passport is required and a visa may also be required. No vaccinations are required. Children under 18 years of age must obtain an un-abridged birth certificate to travel.
